To use this service, please contact the surgery for your unique log-on ID, password and instructions on how to use the service. Alternatively you can complete the necessary forms below. You should make yourself familiar with the information leaflet before starting the application process.

Did you know you can now view a summary of your medical records online via Patient Access?
To activate the online medical records viewing functionality within Patient Access for new and existing users. Firstly you will need to come in to reception and ask for this to be activated, you must provide Photo ID for the receptionist to confirm your Identity for us to be able to do this for this reason we cannot activate this service over the telephone. This is so that your confidential medical records are not accessed by anybody but the intended person - yourself.
If you are experiencing issues with Patient Access and need help please read the following page https://support.patient-access.co.uk/ for Frequently Asked Questions (FAQ’s) as the practice will only be able to issue you with your login details and cannot provide support for how to use these services we also cannot guarantee that the Patient Access service will always be 100% functional as they are outside of control of the practice, the services are hosted securely to NHS and government standards and supplied by our medicals systems provider E-MIS. In cases where patients are having difficulties with online access you can always choose to use another method to submit your repeat prescriptions or to book appointments until the issues are resolved.
