New Patient Registration

Alternatively you can register with the practice by downloading and completing this form PRF1 Form and bringing it into reception or sending it to the practice via email to: surgery.wheatbridge@nhs.net 

Practice Area

Registrations from patients living within our practice area are welcome. However, the practice cannot register patients living outside this area. Existing patients moving out of the boundary may be asked to register as an Out of Area registered Patient, but if these forms are not completed or your GP does not think this service is appropriate you will be asked to sign up with another GP.
