Annual Review
Each year we invite our patients with long term conditions for an annual chronic illness review. This is for a range of conditions, including, but not limited to:
- Asthma / Chronic Obstructive Pulmonary Disease (COPD)
- Cardiovascular/Heart Disease including Atrial Fibrillation, Heart Failure and High Blood Pressure (Hypertension)
- Chronic Kidney Disease (CKD)
- Diabetes
- Dementia
- Learning Disabilities
- Mental Health
- Rheumatoid Arthritis
- Stroke and Transient Ischaemic Attacks (TIAs)
An Annual Review is required for all patients with a chronic disease. The purpose of an annual review is to ensure patients are taking the right medication and receiving the best possible care for their condition(s).
We encourage our patients to take an active role in helping to manage their own conditions, but it is helpful to attend the practice at least once a year for a review.
The majority of our annual reviews are carried out by our nursing team. Depending on your condition you may need 2 appointments: one with a Healthcare Assistant and one with the Practice Nurse. The reception team will advise accordingly and book your appointment(s) with the most appropriate clinician(s).
Before Your Appointment
- Diabetes/CKD: If you also suffer from Diabetes /Pre-Diabetes or CKD please make sure you bring a urine sample to your appointment. Sample bottles can be picked up from reception if needed.
It is important to attend your review appointment so we can ensure your condition is properly controlled and managed. If you run out of your repeat medication and have not had a review, we may not be able to re-authorise your repeat medication.
